Directory Form

What is it? The DMS PTSA Directory is a tool for our families that helps our community stay in touch. This directory is for the sole use of students, their families, and the faculty/staff at Dumbarton Middle School. Student data is taken from this form and NOT from school files. Therefore, students at Dumbarton must be added to the PTSA Student Directory using this form.

How do I access the directory? We are switching to an online directory this year. Online directory access will be included with your PTSA membership ($10 individual/$20 family). Online access will be password-protected and will not be available to the public.
